Please see the attached document for the 25-26 AIA Athletic Physical Form. Please remember we will need all 8 pages signed, completed, and uploaded to your BigTeams registration account prior to clearing your athlete for participation during the 25-26 academic school year.
Physicals must be completed on or after 3/1/25 to be acceptable for the 25-26 school year.
